What do you need to prepare in applying for a VISA?

PRIMARY REQUIREMENTS

Note: All documents must be submitted in A4-size bond paper except for original documents.

• Original Passport (Old and New)

• Passport size picture (2 pcs, must be taken from photo studio, with formal attire and have white background)

• Travel History (photocopy all passport pages with stamps, visa stickers, and e-visa 

• Accomplished Application Form

  • Fill out the application form accurately and completely. 

• Original Latest PSA Birth Certificate

  • Birth certificates must be issued on security paper (SECPA) by the Philippine Statistics Authority. Please visit either PSA  Helpline or PSA Serbilis for further information. 

• Original Latest PSA Marriage Certificate *if married

• If your husband/wife is deceased, kindly provide Death Certificate

• Annulment/Divorce papers (if applicable)

• Photocopy of Valid and/or Government ID (back-to-back)

PROOF OF FUNDS

• Original Bank Certificate (ideal amount for current balance is Php 300,000 while the ideal ADB is Php 200,000)

• Note: The Bank Certificate must be addressed to the Embassy you are applying with  in the Philippines 

• Original Bank Statement (6 months)

ADDITIONAL DOCUMENTS

• Land Titles

• Official Receipt (OR) and Certificate of Registration (CR)

• Credit Card (front only)

• Latest 3 months billing statement of Credit Card
